On this 22nd day of March 1883 personally appeared B. W. Dakin whose post
office address is Callerad Owen County Indiana

Well known to me to be reputable and entitled to credit, and who being duly
sworn according to law upon his oath declares that he was well acquainted
with Isaac S. Poe of Company "E" Twelfth Indiana Volunteers, from the summer
of 1865 to the fall of 1869. And he further declares that said Isaac S. Poe
did suffer with Chronic Diarrhea, and heart trouble, deponent further
declares that said Isaac S. Poe did loose about one third of the time that
deponent knew said Poe as above stated from the disease and heart troubles
as above stated.
